PLAN DOCUMENTS
As participant in the various fringe benefit funds, you are entitled to receive plan documents for each fund. You should download a copy and keep with your other personal documents. You may also obtain a copy from the Fund Office.
Forms
The forms below are important documents needed for your benefits. If you have not completed one of the forms, you should do so as soon as possible.
DC Elective Deferral
This form is needed yearly for those Apprentices on health coverage (2nd - 5th year)
This form allow those Apprentices on Health coverage to designate a beneficiary to receive the Death Benefit should you pass away while on active coverage.
This form is utilized to elect to have money take out of your gross pay to increase your DC Fund account balance. DC Fund is available for 4th & 5th year Apprentices.
Election Period is October thru November each year.
This form allow those Apprentices (4th & 5th year) who are vested in their DC Account to designate a beneficiary to receive the Death Benefit should you pass away prior to retirement.
If you move, this form will give the Fund Office your new address. Completing this form ensures you receive any & all important communication from the Benefit Funds.
This form allows you to designate a beneficiary to receive your vested benefit should you pass away prior to retirement.
This form allow those Apprentices on Health coverage to enroll a new spouse or dependent on to health coverage. Documentation is needed.
